Requesting a Wig

Hello,

My name is Wilma Richards, and I am one of the volunteers and founder of the Butterfly Club. We are a non-profit 501(c)3.

We are sorry and we understand what you are going through, as many of us are also cancer survivors.

To request a wig:

These are the REQUIREMENTS that the executive board has set in place, I need to receive the following information from you:

1. A picture with ALL of your hair gone
2. A picture of your port or pic line
3. A picture of your face with the port
4. A letter with your medical diagnosis
5. A picture of what kind of wig you'd like 6. Current mailing address and telephone number (with FaceTime/WhatsApp)

Once I receive ALL the above, you will get an email with a date and time YOU can call us via Whatsapp or Facetime.

These requirements are in place to avoid fraudulent requests.
We will NOT respond to any telephone calls, or emails that do not include all the above.

Wigs will be selected and mailed out on the third Thursday of each month.
